Femto-LASIK (Femto laser-assisted in situ Keratomileusis) eye surgery is a proven method of refractive surgery for the correction of a visual disorder. In contrast to PRK, a Femto-LASIK surgery creates a corneal flap. This means that the top layer of the cornea is not removed, but rather folded to the side like a small door.

After the LASIK surgery, the flap is put back again and acts like a plaster protecting the lasered eye. This results in an especially painless recovery time. By the next day, the small flap has already grown back, and the surface of the eye is intact again. In contrast to the PRK, you will have almost reached full visual acuity with Femto-LASIK by the very next day.

Differences between LASIK surgery and Femto-LASIK surgery

In essence, LASIK (laser-assisted in situ keratomileusis) with or without femtosecond laser is the same surgery. In both methods, a small flap is created from the uppermost layer of the cornea. This is folded to the side like a small door so that the visual problem of the eye can be corrected underneath. After the laser eye surgery, the flap is put back and will grow again on its own.

With conventional LASIK, the flap is manually detached from the rest of the tissue with a microkeratome – a tiny blade.In contrast to classic LASIK, the flap in Femto-LASIK is generated by the femtosecond laser*. The laser generates tiny gas bubbles under the uppermost layer of the cornea through tiny impulses so that it can be lifted. This only takes a few seconds.

*A femto second is a quadrillionth of a second.

Femto-LASIK is better! Why?
  • The femtosecond laser is more precise than even the calmest surgeon’s hand, making it a bladeless LASIK.
  • Tiny gas bubbles gently release the tissue layers from each other and unlike the blade, exert no pressure on the surrounding tissue.

Femto-LASIK is a higher-value, scientifically recognized, modern and bladeless LASIK surgery.

Moreover, the femtosecond laser is not merely an alternative. It is a procedure that is also recognized as adding independent medical value: Amtsgericht Reutlingen 2015 (in German).

Lasermed-standard: wavefront LASIK

In addition to nearsightedness, farsightedness and astigmatism, your eye may also have other small optical aberrations. These can be traced back to small variations of the cornea and/or the lens. These variations can influence the results of a Femto-LASIK surgery and are determined by a wavefront analysis in the preliminary examination at the vision institute.

The wavefront analysis sends a light into the eye and measures how the light exits the eye again. Ideally, an error-free wavefront should be created. If this is not the case, the corresponding deviations can be considered with the wavefront optimization – an additional function of the laser.

Wavefront optimization makes the result of laser eye surgery even more precise. This avoids the risk of side effects such as glare sensitivity halos and improves vision at dusk. Using a wavefront optimization especially for those with relatively large pupils is advantageous.

Femto-LASIK

The surgery

After a free consultation at the vision institute, you will come for a preliminary examination with one of our ophthalmologists. In the preliminary examination, your eyes will be measured and examined again in detail. You will also be informed about the process of LASIK surgery and possible risks. Once everything is approved, you will receive an appointment for your LASIK surgery. On the day of procedure, our experienced team of optometrists and doctors will assist and safely accompany you at every step.

Laser Eye Surgery

  • Before the laser eye surgery, your eyes will be anaesthetized with an eye drop medication so that the procedure is completely painless.
  • The femtosecond laser is then used to form the so-called flap and is folded to the side. The formation of the flap is completely painless. The flap (also called a lamella) is like a little door in the corneal surface behind which will be lasered.
  • The excimer laser is then used to correct your visual disorder – this process takes only a few seconds and is painless.
  • After LASIK eye surgery, the flap is put back again. Shortly after the surgery, you can already see much sharper –without glasses!
The preliminary examination
Measuring your eyes
  • After you have decided that LASIK eye surgery with Femto-LASIK is right for you, make an appointment for a preliminary examination in one of our vision institutes.
  • In order to ensure correct measurements, please do not wear soft contact lenses for two weeks and hard contact lenses for four weeks before the preliminary examination.
  • In the preliminary examination, your eyes will be comprehensively measured again by one of our optometrists. One of our ophthalmologists will then examine your eyes again and, as with every surgery, will explain the procedure, possible risks and side effects.
  • You can expect the following eye measurements in the preliminary examination:
    • Determination of eye length
    • Measurement of anterior chamber depth
    • Measurement of pupil width
    • Determination of refractive values in dioptres and additional subjective adjustment
    • Control for anterior and posterior segments of the eye
    • Control for eye pressure
  • Femto-LASIK requires a sufficiently thick cornea (at least 500 μm).
COSTS OF THE PRELIMINARY EXAMINATION
  • The preliminary examination is carried out by one of our ophthalmologists and is therefore a medical service with a fixed cost pre-determined by law. The billing is carried out according to what is called the Gebührenordnung der Ärzte (GOÄ), or fee regulations for physicians in Germany. However, the costs of the preliminary examination in the vision institute are included in the LASIK eye surgery costs.

What you need to keep in mind
  • You should be accompanied on the day of your surgery, as you are not allowed to actively participate in traffic After a laser eye surgery, your eyes will be covered with one opaque bandage and one transparent bandage each, limiting your vision.
  • You are not allowed to wear make-up, perfume, or hairspray on the day of surgery.
  • Feel free to eat and drink normally before the surgery.
The preparation
  • Before the procedure starts, you will be given a light sedative so that you can stay relaxed as you start your new life without glasses. Your eyes will then be anaesthetized with corresponding eye drops. Laser eye surgery is therefore completely painless.
  • You lie down on the surgery chair. The eye laser is above your head, the ophthalmologist sits at the direction of your head. A small clamp keeps your eye open so you don’t have to worry about accidentally blinking.

Laser Eye Surgery
  • The first step in treatment with Femto-LASIK is creating the corneal flap. A femtosecond laser inserts small gas bubbles into the tissue with micrometer precision, separating the flap by increasing the volume. The flap works like a small door, behind which the laser is applied.
  • The flap is already formed after 17 seconds. A small suction ring ensures the central position of the flap. You feel the attached ring in the form of a firm pressure on the eye for only 30 seconds.
  • The second step is to correct the visual disorder. The surgeon carefully lifts the flap and folds it to the side. Now the area being treated is ready for the second laser – an excimer laser. This only lasts 2-3 seconds per diopter – you won’t feel anything.
  • During the surgery you will look at a small light above you. Rest assured, you can’t do anything wrong and do not have to be afraid of accidentally moving your eye. The laser can easily compensate for involuntary eye movements with eye tracking. If the movements become too large, the laser stops immediately. The Femto-LASIK can continue on again smoothly after a short restart. The laser cannot “slip”.
  • Following the laser procedure, the corneal lamella (flap) is returned to its previous position by the surgeon. Here, it is embedded in the surrounding corneal tissue and grows back on its own. It acts like a plaster, so the recovery process is almost painless.
  • The LASIK eye surgery is completed after about 20 minutes.
Directly after Laser Eye Surgery
  • Immediately after laser eye surgery with Femto-LASIK, your vision will still be slightly blurred. On the first day after the laser eye surgery however, your vision is almost completely sharp.
  • For protection, your still very sensitive eyes are blindfolded. An eye patch made of transparent plastic is put over one eye so that you can still see, but with some restrictions. For this reason, you should arrange for someone to take you home.
  • It is best to spend the evening in dim light, without screens, or simply to go to bed early in order to let your eyes recover.
  • In the following days, the only medication you will take are eye drops according to a prescribed eye drop schedule that prevents inflammation.
  • Already from the first day after the laser eye surgery, you can enjoy your life without glasses.
After the surgery
THE TIME AFTER FEMTO-LASIK EYE SURGERY
  • We don’t take any risks. After LASIK surgery, you will come 1 day, 1 week, 1 month and 1 year after for follow-up examinations. This way we can be sure that your eyes will recover safely.
  • Already from the first day after the surgery, you will have almost reached your complete visual acuity but with a slight sensation of a foreign substance in your eye. If, for example, you work in an office setting, you can even go back to work the next day.
  • You will notice that your eyesight will improve day by day and that you will not feel the side effects of the surgery after a few days. However, you should not wear make-up or rub your eyes for the first few days after surgery.
  • In the weeks that follow, you may notice that you are somewhat sensitive to light or have dry eyes. Sunglasses and eye drops will help against these side effects.

Femto-LASIK is the most frequently used LASIK eye surgery. It is suitable for the majority of all patients and is particularly painless due to the generation of a corneal lamella (the flap). Due to this so-called flap, it allows you to see sharply the next day and go back to work. The recovery is very short and uncomplicated. What is Femto-LASIK eye surgery?

Femto-LASIK (Femto-laser-assisted in situ Keratomileusis) is a proven procedural method of refractive surgery for the correction of vision disorders (ametropia). In contrast to PRK/LASEK, Femto-LASIK eye surgery uses the femtosecond laser to form a small corneal flap. The actual laser correction can then be carried out under the flap. The flap is then covered and acts like a plaster so that a particularly fast visual experience is achieved.

What does Femto mean?

“Femto” is a unit of the metric system. A femtosecond is a quadrillionth of a second, written 0.000,000,000,000,000,001 seconds. Femtosecond describes the speed with which the pulses of the laser are emitted. The laser therefore works extremely fast, so that the flap is formed within only 17 seconds.

How does Femto-LASIK surgery work?

Femto-LASIK (Femto-laser-assisted in situ Keratomileusis) involves the formation of a small corneal lamella (the so-called flap) from the uppermost layer of the cornea (the epithelium) using a femtosecond laser. The flap is then carefully folded to the side. Under the flap, the visual problem is corrected with the excimer laser. This changes the surface of the eye so that the light in the eye is refracted optically correct and that you can see sharply without glasses. The flap is then put back again and grows within the next hours by itself. It works like a plaster produced by the body and makes it possible for you to see relatively sharp directly after LASIK eye surgery. Due to the creation of the flap, the recovery is painless. You only have a slight feeling of a foreign substance in your eye.

Femto-LASIK vs PRK, which is better?

In general, there is no better or worse to the Femto-LASIK vs PRK/LASEK question. The individual condition of the eye will ultimately determine the procedure that is best suited for each LASIK eye surgery candidate personally. We will be happy to advise you personally on this in a free, initial consultation. In general, Femto-LASIK is a gentler LASIK surgery due to the formation of a flap and you can see sharply much faster following the procedure. However, the PRK/ LASEK is better suited for LASIK eye surgery candidates with particularly thin corneas. If you play any contact sports or work for the police force, fire department or military, Femto-LASIK is not recommended due to the flap.

What are the advantages / disadvantages of Femto-LASIK surgery?

Due to the formation of a flap (a small corneal lamella), Femto-LASIK surgery is particularly gentle and painless. Since the flap seals the eye surface like a plaster, you can see relatively sharp directly after the LASIK eye surgery and go back to work (in an office setting) the next day. The recovery isn’t painful.
Femto-LASIK is not suitable for LASIK eye surgery candidates with thinner corneas. It is also not recommended for those who play contact sports, are police officers or firefighters due to the flap.

When can I see sharply again after Femto-LASIK surgery?

After a Femto-LASIK surgery, you can already see directly after the procedure. Your vision improves significantly in the following hours. So much that on the following day, you can already see relatively sharp. Full visual acuity is achieved after 3-4 days.

How long is the recovery time after Femto-LASIK surgery?

Due to the flap, the recovery time after a Femto-LASIK surgery is very fast. It takes two weeks for the eye to heal completely, but the flap will have already grown back within a few hours after the surgery. Your vision improves very quickly, so that the only side effects you notice are primarily in the first days such as a slight feeling of a foreign substance, dry eyes and sensitivity to light. In addition, the recovery time is painless, the flap acts like a body’s own plaster.

When can I exercise or play sports again after Femto-LASIK surgery?

As a rule, you can play sports again two weeks after Femto-LASIK surgery. Until then, you should give your eyes time to heal after the procedure and avoid playing sports to prevent sweat or dirt from getting into your eyes.

When can I drive again after Femto-LASIK surgery?

As a rule, you can drive a car again a few days after Femto-LASIK eye surgery. This can vary from person to person but will be determined more concretely in the follow-up examinations. You will receive a certificate that you are allowed to drive without visual aids.

When can I wear make-up again after a Femto-LASIK surgery?

In order to avoid the risk of dirt particles getting into your healing eye, you should refrain from wearing any make-up two weeks after Femto-LASIK surgery. Applying make-up and removing make-up inevitably leads to small bits of make-up getting in your eye. This increases the risk of infection. In addition, rubbing yours eyes during the recovery phase can cause the flap to slip. Two weeks after the procedure, you can wear make-up again as usual.

How long will I see halos after Femto-LASIK surgery?

After a Femto-LASIK surgery, small rings of light known as halos can be seen around light sources such as streetlamps. However, not everyone sees halos because of Femto-LASIK. If they do show up, they usually disappear quickly in the first weeks after the surgery.

How painful is Femto-LASIK surgery?

Before the surgery, your eyes will be anesthetized with an eye drop medication so that the LASIK surgery is completely painless. Since the flap generated by Femto-LASIK surgery acts like a plaster produced by the body, the recovery time after the surgery is painless. You may only have a slight sensation of a foreign substance in your eye, but this will quickly disappear again.

How long do I need to use eye drops after Femto-LASIK surgery?

Following Femto-LASIK surgery, your eyes need to be treated intensively with antibiotic eye drop medication several times a day for a week. The eye drops help the eye to recover and prevent infections. After the LASIK surgery, you will receive a prescribed eye drop schedule indicating the intervals and timetable to use the eye drops. If you stick with it, you can’t do anything wrong. If necessary, you can also use moisturizing eye drops to reduce dry eyes.

What is a femtosecond laser?

The femtosecond laser is a laser used in refractive surgery for Femto-LASIK in order to prepare the eye for the actual laser eye surgery. The femtosecond laser generates a small lamella from the upper layers of the cornea – the so-called flap. The laser gives impulses to the eye at the speed of one quadrillionth of a second. The impulses from the laser cause tiny gas bubbles to form between the cell layers of the cornea. The layers become separated through the volume of the bubbles created. LASIK surgery with femtosecond laser is much gentler, and more precise. It is also a bladeless LASIK, unlike conventional LASIK surgery in which the flap is formed with a microkeratome (a small blade).

How many dioptres are treated with Femto-LASIK surgery?

With Femto-LASIK, Lasermed corrects nearsightedness (myopia) up to -7 dioptres, farsightedness (hyperopia) up to +4 dioptres and astigmatism up to +4 dioptres. According to the guidelines issued by the Commission for Refractive Surgery (in German, Kommission für Chirurgie – KRC), values up to -10, +4 and +6 dioptres can be corrected with Femto-LASIK surgery. At Lasermed, however, we only treat visual disorders according to the limits that we have set here at our vision institute. This is due to the fact that, according to our own experience, the risk of side-effects increases and can lead to dissatisfaction and lower quality if we exceed our pre-determined values. For those with higher dioptres values, our experience would recommend other alternative procedures which are more medically sound. When should I stop wearing contact lenses before Femto-LASIK surgery?
You should not wear contact lenses on the day of Femto-LASIK surgery. In the days leading up to the surgery, you may still wear your contact lenses as usual.
