The implantable Collamer® lens EVO Visian ICL™ from Staar Surgical company is a modern and particularly gentle method for correcting vision problems. In contrast to Femto-LASIK and PRK/LASEK, the corneal surface is not irreversibly changed Instead, an additional artificial lens made out of collamer is implanted into the eye. The EVO Visian ICL procedure is situated invisibly between the patient’s own natural lens and the iris.

EVO Visian ICL™ vs. multifocal lens

The EVO Visian ICL is a so-called phake (in Greek “lens”) intraocular lens (PIOL) which is implanted into the eye (intraocular) in addition to the body’s own natural lens. With the implantable collamer lens, more severe visual disorders can be corrected than with laser eye procedure: Nearsightedness up to -20 diopters and farsightedness up to +10 diopters, and with a toric lens, an astigmatism up to -6 diopters. For lens implantation, you must have sufficient anterior chamber depth.

Since the body’s own natural lens has not yet lost its ability to adapt (presbyopia, also known as age-related vision disorders), the implantable contact lens (ICL) is a monofocal lens instead of a multifocal lens. Unlike a trifocal or multifocal lens, it has only one focal point. A multifocal lens, on the other hand, is used for lens replacement (CLE) or cataract surgery.

The Procedure

After a free, initial consultation, your eyes will be precisely measured again during the preliminary examination. This includes, among other things, determining the anterior chamber depth to ensure that there is sufficient space in your eye for the EVO Visian ICL. Once this is done, the appointment for your big surgery day can be arranged. We then place the order for your EVO Visian ICL lenses, which – just like glasses – are individually customized for you.

The Lens Implantation

  • Before the implantable collamer lens can be inserted, your eye will be anaesthetized with eye drops and the pupil will be dilated. On request, you can be given a monitored anesthesia.
  • A small opening (2.2 mm) is then made at the edge of the cornea. This is completely painless due to the anaesthesia.
  • Finally, the EVO Visian ICL lens is inserted through the opening. The folded collamer lens then unfolds and aligns in the eye by itself. This is controlled by the surgeon.
  • Within the next 48 hours, your vision will steadily improve. You won’t have any pain.
The preliminary examination
The measurement of your eyes
  • Once you have decided on an implantable collamer lens after the first, free consultation at our vision institute, make an appointment for a preliminary examination.
  • Before the preliminary examination at the vision institute, you are not allowed to wear soft contact lenses for two weeks and hard contact lenses for four weeks in order to guarantee correct measurements.
  • In the preliminary examination, your eyes will be measured again accurately and comprehensively by one of our qualified optometrists. Your eyes will then be examined again by an ophthalmologist and you will be informed about the risks and side effects.
  • You can expect the following measurements in the preliminary examination:
    • Determination of eye length
    • Measurement of anterior chamber depth
    • Pupil width measurement
    • Determination of your refractive outcome values in diopters and additional subjective adjustment
    • Control of the anterior and posterior segments of the eye
    • Eye pressure control
  • Especially important for the implantable collamer lens is a sufficient anterior chamber depth to ensure that there is enough space in your eye for the EVO Visian ICL.
COSTS OF THE PRELIMINARY EXAMINATION
  • The preliminary examination is a medical service with a fixed cost being pre-determined by law. However, the costs of the preliminary examination are included in the surgery costs. If you later decide against an EVO Visian ICL lens implantation, the costs of the preliminary examination must still be paid. Usually, they are not covered by health insurance companies.
Before surgery
What you need to keep in mind
  • On the day of the surgery, you and the person accompanying you should come together to the LASERMED vision institute. After the lens implantation, your vision will still be restricted. In addition, you will also get an eye bandage and are not allowed to actively participate in traffic.
  • Do not wear make-up, perfume or hairspray on the day of lens implantation.
  • For your surgery appointment at the vision institute, you must be sober and must not eat or drink beforehand.
The preparation
  • Before you start your life free of glasses with the EVO Visian ICL, you will be given a light sedative and, if you wish, a monitored anesthesia—as to let the implantation happen in a relaxed state. In addition, your eyes will be anesthetized with eye drops so that the procedure is completely painless.
  • For the surgery, you will be given a pair of overalls that you simply pull over your own clothes. Our experienced team will then accompany you to the sterile surgery room.
  • You lie down on the surgery chair and your eye surgeon will be sitting at the direction of your head. During the surgery, you don’t have to worry about keeping your eyes open or blinking accidentally as a small clamp takes care of that for you.
 
More details on the implantation of the EVO Visian ICL™
CONTACT LENS IMPLANTATION
  • In contrast to classic laser eye surgery with PRK/LASEK or Femto-LASIK, both eyes are not treated in one day with implantable contact lenses for safety reasons. This is because the implantation of the EVO Visian ICL takes place deeper in the eye in comparison to LASIK surgery. To this day, Lasermed has had no complications with the implantation of the EVO Visian ICL. Nevertheless, we would like to make sure that your one eye is completely intact before we treat the second.
  • Before the EVO Visian ICL can be implanted, the surgeon makes two small incisions at the point where the transparent cornea transitions into the whitish sclera. The incisions are only 2.2 mm in size and do not have to be sutured afterwards. Thanks to the previously given anesthesia, this is completely painless.
  • The EVO Visian ICL is then put through the small slit in the eye surface. The lens is located in a viscous substance and is inserted into the eye folded through a small tube. It is placed between the iris and the eye’s own natural lens where there is sufficient anterior chamber depth and unfolds by itself. The correct placement is checked again by the surgeon.
DIRECTLY AFTER CONTACT LENS IMPLANTATION
  • After the lens implantation, you will see much sharper with the treated eye. Unlike PRK/LASEK or Femto-LASIK, the cornea of your eyes remains untouched and does not have to recover. Nevertheless, your brain has to adjust to the new abilities of your eyes, so it takes a few days until you have reached full visual acuity.
  • For protection, your eye is blindfolded which is why your vision will be limited to some extent after the surgery and is why you should have someone accompanying you home.
  • In the days following the lens implantation, you will take eye drops according to a prescribed eye drop schedule. This will prevent inflammation, thus already eliminating the greatest risk of EVO Visian ICL.
  • On the following day, we will remove the bandage for you in the follow-up examination. You will notice that your vision has already increased significantly.
After the surgery
YOUR LIFE WITH THE EVO VISIAN ICL
  • After the implantation of the EVO Visian ICL, you come for follow-up appointments in intervals of 1 day, 1 week, 1 month and 1 year. In this way, we can ensure that your eye recovers optimally.
  • Your vision will be significantly better directly after the procedure and will continue to improve day by day. Generally speaking, you will have achieved full visual acuity after 1-2 weeks. However, this may vary from patient to patient.
  • In contrast to classic laser eye surgery, you will experience dry eyes significantly less during the recovery time but will be just as sensitive to light. Eye drops can be soothing in the beginning so you can very quickly enjoy your life without glasses.

What does ICL mean?

ICL stands for Implantable Collamer Lens. The ICL is therefore an additional implantable contact lens made out of collamer that is implanted into the eye to correct a visual problem. The collamer lens is inserted as an addition into the posterior chamber of the eye—between the iris and in addition to the eye’s own natural lens. In contrast to classic laser eye surgery (using PRK/ LASEK or Femto-LASIK), the ICL procedure is minimally invasive and completely reversible as nothing is removed from the cornea and the lens can be removed at any time without leaving a trace. In addition, with the EVO Visian ICL you can correct almost any visual problem and you can already see completely sharp after the lens is inserted.

How many diopters can be corrected with EVO Visian ICL?

In contrast to classic laser eye surgery with PRK/LASEK or Femto-LASIK, significantly higher values can be treated with EVO Visian ICL. The EVO Visian ICL can correct astigmatism up to 6 diopters, farsightedness with up to 10 diopters and nearsightedness with up to -20 diopters. In addition, the thickness of the cornea is irrelevant for EVO Visian ICL. Only a sufficient anterior chamber depth is necessary.

How much does an EVO Visian ICL lens cost?

When will I have full visual acuity with an EVO Visian ICL?

Vision is usually achieved the day after implantation of the EVO Visian ICL.

How long do ICL lenses last?

After implantation, the EVO Visian ICL lens can remain in the eye indefinitely. Current studies show that the material does not decompose even after decades and to this day, Lasermed has had no complications with the implantation of the EVO Visian ICL. Nevertheless, the ICL can be removed at any time without leaving a trace. However, ICL is usually only removed during a cataract operation in old age as there is no longer a need for it.

Can the ICL slip?

No, the EVO Visian ICL cannot slip in the eye. The phake lens is designed to align itself and remain in this position during lens implantation. As it sits behind the iris, it cannot fall out and is not visible. Complications in general are extremely rare.

What are implantable collamer contact lenses?

Collamer lenses allow the correction of many—especially severe—visual problems. Similar to a contact lens, the correction is produced by the effect of the lens. This is located behind the iris and is invisible. With ICL, vision disorders can be corrected of up to +6 diopters for astigmatism, up to +10 diopters for farsightedness and up to -20 diopters for nearsightedness. ICL is therefore a smart alternative if classic LASIK surgery is not desired or not possible.

What are the pros and cons of an implantable collamer lens (EVO Visian ICL)?

In general, the implantable collamer lens (EVO Visian ICL) only has advantages. The ICL procedure is minimally invasive, completely reversible and particularly gentle, since —in contrast to a classic LASIK surgery—nothing is removed from the corneal surface. In addition, you can see sharply immediately after the lens is inserted. In very rare cases, however, ICL can lead to an early development of cataracts. If this is the case, the ICL is simply removed and the eye’s own natural, clouded lens is replaced by an artificial lens—which again leads to a sharp image.
