PRESBYOND® Laser Blended Vision (presbyopia + beyond) is a form of laser eye surgery. Developed by Carl Zeiss Meditec AG, the procedure is a unique form of Femto-LASIK in treating patients who have vision problems due to ageing eyes, also known as presbyopia.

Similar to the conventional monovision method, the dominant eye is corrected for distance and the non-dominant eye for proximity. For most people, the brain can combine the different images of both eyes into one image, so you no longer need reading glasses. This type of LASIK surgery allows you to see sharply and with high contrast for near and far ranges, as well as in the intermediate range (e.g. working on a PC). This so-called “blend zone” is determined individually for you.

Differences between PRESBYOND & Monovision

On a fundamental level, both PRESBYOND and conventional monovision are methods of refractive surgery used to correct presbyopia. In both procedures, the dominant eye is corrected for distance and the non-dominant eye for proximity. Reading glasses therefore become unnecessary.

Monovision

*The intermediate range lies between the near and far range. This is, for example, the distance in a personal conversation or working on a computer screen.

Multifocal lens

In addition to PRESBYOND or monovision, your visual problem and/or presbyopia can also be corrected with a multifocal lens. A multifocal lens is an artificial lens with several focal points that is inserted into the eye in place of the natural lens through a CLE (clear lens extraction). The use of a multifocal lens is therefore not a laser surgery. The multifocal lens is also used for cataract surgery.

In contrast to the multifocal lens, the PRESBYOND has the advantage that only the surface of the eye (cornea) is treated with the laser.

The Procedure

In general, the laser treatment of your presbyopia with PRESBYOND laser vision correction is just like a Femto-LASIK procedure. The difference lies only in the way the visual disorder is corrected.

Laser Eye Surgery

As with Femto-LASIK, a so-called corneal flap is formed before the actual laser eye surgery. Afterwards, the eyes treated with PRESBYOND are corrected for the corresponding distances with the Zeiss excimer Laser. The only difference from Femto-LASIK surgery is in the variance of correction to the eyes. Thus, the dominant eye is corrected to nearly 0 diopters and the non-dominant eye is under-corrected to -1.5 diopters. Through this, your brain is then able to select the sharpest image for each distance. This means you will no longer need reading glasses and that your presbyopia is a thing of the past.

The preliminary examination
THE MEASUREMENT OF YOUR EYES.
  • If you have opted for a PRESBYOND laser vision correction for your presbyopia, you will need to first make an appointment for your preliminary examination at the eye center.
  • In order for us to guarantee the correct measurement of your values, you are not allowed to wear soft contact lenses for two weeks and hard contact lenses for four weeks before the preliminary examination.
  • Your eyes will be comprehensively measured in the preliminary examination by one of our optometrists. Following the measurements, they are examined by one of our ophthalmologists and you will be informed about the treatment as well as the risks and side effects of the procedure.
  • The following measurements are made in the preliminary examination:
    • Determination of eye length
    • Measurement of anterior chamber depth
    • Measurement of pupil width
    • Determination of your refractive values in diopters and additional subjective adjustments
    • Control of the anterior and posterior segments of the eye
    • Eye pressure control
  • Since PRESBYOND is a Femto-LASIK procedure, you must also have a sufficiently thick cornea (at least 500 μm).
COSTS OF THE PRELIMINARY EXAMINATION
  • Since the preliminary examination is performed by an ophthalmologist, this is a medical service in which the billing is pre-determined by law. However, the costs for the preliminary examination are included in the surgery costs.
Before surgery
WHAT YOU NEED TO KEEP IN MIND
  • Avoid risks: after the PRESBYOND laser vision correction, you are not allowed to actively participate in traffic. For this reason, you should be accompanied to the eye center on the day of the surgery. Moreover, you will be given an opaque and transparent bandage which will limit your vision after the LASIK surgery.
  • Do not wear make-up, perfume or hairspray on the day of the surgery.
  • Feel free to eat and drink normally before the surgery.
The Preparation
  • Before your presbyopia is treated with an eye laser, you will be given a light sedative so that you can start your life free of glasses in a relaxed way. Your eyes will then be anaesthetized with eye drops therefore making the PRESBYOND laser vision correction completely painless.
  • For the procedure, you will be laying down on a surgical chair. The Zeiss laser is above your head, the treating ophthalmologist sits at the direction of your head. During PRESBYOND laser vision correction, you don’t have to worry about blinking accidentally as a small clamp keeps your eyelid open for you.

LASER TREATMENT OF YOUR AGEING EYES
  • In the first step of PRESBYOND laser vision correction, a small corneal flap is created, the same as in normal Femto-LASIK. The femto laser creates small gas bubbles in the tissue with micrometer precision, separating the flap from the eye due to the increase in volume. Finally, underneath the generated flap, as if behind a small door, the eye is lasered.
  • The flap is formed after only 17 seconds. Its central positioning is ensured by a small suction ring. You only feel this for 30 seconds in the form of a firmer pressure on your eye.
  • Your presbyopia is then corrected with After the surgeon has placed the flap aside, the treatment area for the Zeiss excimer laser is ready. Per diopter, it takes only 2-3 seconds during which you won’t feel anything.
  • During the PRESBYOND laser vision correction you look at a small light above you—you don’t have to worry about doing something wrong. The Zeiss laser can easily compensate involuntary eye movements with eye tracking. If the movements are too large, the laser stops immediately. The PRESBYOND laser vision correction can be continued seamlessly after a short restart of the Zeiss eye laser. There is no risk that the laser can “slip”.
  • Following the laser correction, the corneal flap is returned to its previous position. It grows back on its own by embedding itself in the surrounding tissue. The flap acts like a plaster, so the recovery is almost painless. It also reduces the risk of inflammation.
  • The procedure at the eye center only lasts about 20 minutes. Your presbyopia has been corrected and you no longer need reading glasses.
DIRECTLY AFTER PRESBYOND LASER VISION CORRECTION
  • Directly after the PRESBYOND laser vision correction, your vision will still be slightly blurred. Already on the first day after the surgery, the flap has grown back again very firmly, so that you can see sharply again.
  • Your eyes, which are still very sensitive, are bandaged with a normal bandage and a transparent eye patch for protection. This restricts your vision and you should therefore be accompanied home and not leave the eye center alone.
  • It is best to spend the evening in dim light or to simply go to bed early to let your eyes recover.
  • To prevent the risk of inflammation, take eye drops in the following days according to a prescribed eye drop schedule.
After the surgery
THE TIME AFTER PRESBYOND LASER VISION CORRECTION
  • After PRESBYOND, you will come to the eye center for follow-up appointments in intervals of 1 day, 1 week, 1 month and 1 year. This allows us to ensure that your eyes recover safely and without risk.
  • You will notice your vision improving day by day and that you will not feel the side effects of the procedure after just a few days. However, you should not wear make-up or rub your eyes for the first few days after the surgery.
  • In the weeks following the laser surgery, you may still be somewhat sensitive to light or have dry eyes—sunglasses and eye drops will help. You no longer need reading glasses!

Do you need reading glasses after a monovision procedure?

By having a monovision procedure, you can expect a general freedom from wearing glasses. For reading particularly small texts (e.g. packaging instructions), there is still a chance you may need reading glasses.

How much does a monovision procedure cost (using Femto–LASIK)?
